Issues & Corrective Actions
Learn how to create, track, and resolve issues and corrective actions in Forkto to maintain food safety standards.
The Issues feature in Forkto helps you track problems and corrective actions across your business. Issues can be created manually or generated automatically when something goes wrong — like a temperature reading that’s out of range or a failed intake check.
Overview
When you open the Issues page, you’ll see a table of all issues with their title, status, who they’re assigned to, and when they were created. Use the search bar to find specific issues, and the ‘Create Issue’ button to log a new one.
Each issue also appears on the assigned person’s Home page To Do List with a due time, so nothing gets missed.
Creating an Issue
To log a new issue:
- Click ‘Create Issue’.
- Fill in the form:
- Title (required) — a short summary of the issue.
- Description (required) — what happened and what needs to be done.
- Action Type (required) — choose Corrective Action or Preventative Action.
- Assigned To (required) — select the team member responsible.
- Status — Draft, In Progress, or Resolved.
- Origin Type (required) — categorise the issue: Hygiene, Structural, Procedural, Training, Equipment, Maintenance, or Utility.
- Due Date and Time (required) — when the issue needs to be resolved by.
- Location (required) — which location the issue relates to.
- Click ‘Create Issue’ to save.
Using Templates
For common issues, you can select a template from the dropdown to pre-fill the form. Templates include things like “Hand Washing Violation”, “Equipment Problem”, or “No Hot Water” — each with a sensible default due time.
Issue Statuses
Issues move through three statuses:
- Draft — logged but not yet being worked on.
- In Progress — someone is actively dealing with it.
- Resolved — the issue has been fixed and a corrective action recorded.
Resolving an Issue
To resolve an issue:
- Open the issue from the list.
- Click ‘Resolve Issue’.
- A form will ask you to describe the corrective action you took — this is required.
- Submit to mark the issue as resolved.
If an issue needs to be reopened, click ‘Reopen Issue’ on the resolved issue to set it back to In Progress.
How Issues Link to Other Features
Issues can be created automatically from other parts of Forkto:
- Temperature Logging — when a temperature is logged out of range, you can choose “Submit & Create Issue” to automatically generate a corrective action issue linked to that reading.
- Meat / Ingredient / Packaging Intakes — issues can be raised during the intake process if something doesn’t pass inspection.
When an issue is linked to another feature, the Origin section on the issue detail page shows the full context — the temperature reading, the intake details, or whatever triggered it.
Activity Timeline
Each issue has an activity timeline showing:
- Comments — team members can add, edit, and delete their own comments.
- Change log — every change is automatically tracked (who changed what field, when, and the old vs new value).
This gives you a full audit trail for compliance purposes.
Auditor Access
All users with the Issues feature enabled can view, create, and manage issues. Auditors without the feature flag cannot access this section.
Stay on top of issues by resolving them before their due date. Use the activity timeline to document corrective actions and keep a clear audit trail for inspections.
